When you enroll in your in your Flexible Benefits this is your chance to select the coverage you need most and make the most of your flex dollars by declining coverage you don’t need. Remember, excess flex dollars go to a personal or health spending account or your RRSP if it’s during Annual Enrollment - more funds for your everyday needs!

** Physical services include Physiotherapist, Athletic Therapists, Kinesiologist, Occupational Therapist, Massage Therapist, Chiropractor, Podiatrists or Chiropodists, Osteopaths.
*** Holistic services include Naturopaths, Acupuncturists, Dieticians, Homeopaths, Speech Therapists, Audiologists.
**** Psychological services include Mental Health Practitioners, Clinical Counsellor Clinical Therapists, Marriage & Family Therapists, Mental Health Counsellors, Psychiatrists, Psychoanalysts, Psychotherapists, Psychologists and Social Workers.
***** Fertility coverage includes IVF, IUI, Pre-implantation genetic testing and cryopreservation including storage cost.
****** Coverage includes basic surgical procedures not covered under provincial plan and surgical procedures to align feminine or masculine features to the transitioned gender, such as facial bone reduction or cheek augmentation.

Reimbursement rates and maximums: are dependent on the level of coverage chosen for that plan year. The extended health care plan will reimburse your eligible expenses at the percentages shown in the chart. If you cover dependents, the maximums are per covered person. Extended health care services and supplies are subject to Sun Life’s Reasonable and Customary charge limits.

Reasonable and customary charges:
  • Fees and prices normally charged in the regional area where the services or supplies are provided.
  • Charges for services and supplies that represent reasonable treatment, considering the duration of services and how frequently services and supplies are provided.
You have four different dental options to choose from. If you choose dental Option 3, there is a two-year lock-in period. This means you are unable to change this coverage for two years.

* A dental fee guide contains a summary of suggested charges for all dental services, set annually by provincial dental associations and used by Sun Life to determine the reasonable and customary charge for a service.
SLB Flex provides automatic basic employee life insurance as well as additional optional coverage for you and your family.

Accidental death and dismemberment (AD&D) insurance provides protection for you and your family in the event of accidental death or injury. SLB Flex provides automatic basic AD&D insurance for you, and you may choose to purchase additional optional coverage for yourself and your family.

Critical illness insurance provides you with a one-time, tax-free, lump-sum cash payment that you can use in any way if you or your covered dependent is diagnosed with a covered condition.

Short-term disability (STD) coverage provides you with income protection in the event of a short-term illness or injury; however, there are some differences in coverage, as noted in the table below. Your STD coverage is automatic and fully paid for under SLB Flex.

Long-term disability (LTD) coverage* provides you with income protection should you experience a long-term illness or injury and are unable to work. Payments begin once you are approved for LTD benefits, after your STD benefits have ended.

*Your LTD option is locked in and an Evidence of Insurability (EOI) form is required for LTD increases during Annual Enrollment.

 

SLB provides all employees with additional accident coverage when travelling for business. Your coverage is equal to 4.5 times your eligible compensation to a maximum of $1 million.
