Depending on your province of residence eye exams may be covered under provincial health care for dependents under the age of 18 and for persons 65 years of age or older. Some medically necessary or urgent procedures may be covered as well.

Reimbursement rates and maximums are dependent on the level of coverage chosen for that plan year. The extended health care plan will reimburse your eligible expenses at the percentages shown in the chart. If you cover dependents, the maximums are per covered person. The 24-month period begins from date of purchase.The eye exam reimbursement is also subject to Sun Life’s Reasonable and Customary charge limits.

Reasonable and customary charges:
  • Fees and prices normally charged in the regional area where the services or supplies are provided.
  • Charges for services and supplies that represent reasonable treatment, considering the duration of services and how frequently services and supplies are provided.
